Becoming a member of Empire Mental Health Support gives you the opportunity to keep in touch with all that EMHS is accomplishing. Your membership and support are important as we strive to improve lives.

We recently raised our annual membership fee to $10, but we still feel it's an amount that is reasonable for anyone.

For your convenience we offer several ways to submit your membership.

• Print the membership form, then mail the $10 annual membership fee (plus donation if applicable) to the address below.

• Submit the online form, then mail the $10 annual membership fee (plus donation if applicable) to us.

• Submit the online form, then PayPal us the $10 annual membership fee (plus donation if applicable).​
